Background: Coronavirus disease 2019 (COVID-19) has a variety of laryngeal sequelae. We report a rare case of acquired laryngomalacia (AL) after COVID-19, which was misdiagnosed due to mismatch of the patient's clinical characteristics and epidemiology. Case presentation: A 12-year-old boy with no congenital disease presented with sudden dyspnoea and inspiratory stridor 10 days after a COVID-19 diagnosis. His vital signs were stable, and phonation was normal, but conversation was hampered by breathing difficulty, and inspiratory stridor was heard around the neck. X-ray (chest and neck) and contrast-enhanced neck CT findings were normal. Initial treatments for suspected croup were ineffective, leading to his admission for observation. Laryngoscopy by an otolaryngologist revealed arytenoid-type laryngomalacia. Conclusion: AL has a variety of etiologies and an unclear pathophysiology. AL may be a sequelae of COVID-19, so laryngoscopy should be performed if stridor and dyspnoea after COVID-19 do not respond to appropriate treatment.
